First Visit & New Patient Forms

From the time you step into our office, you will experience the courtesy and professionalism that Crescent Dental is known for.  We value your time and make every effort to make sure you are seen promptly. When you make your first appointment, you have the option of requesting registration paperwork to be sent to you by email or you can download it here at the bottom on this page so you don't have to spend extra time filling out forms when you arrive. If you have dental insurance, we will research your benefits ahead of time before your arrival, so you know what services are covered.

At your first visit, we will take a set of digital photos and x-rays to evaluate your oral health.  Our hygienist will also assess your gum tissues health by performing a complete periodontal charting. If you desire, you may also get your fist cleaning (dental prophylaxis) at this time. The doctor will listen to your concerns and perform a detailed dental and oral cancer screening exam.  Then he/she will review the findings with you.  All aspects of the proposed treatment will be explained to you including number of visits, finances, and insurance coverage. Depending on your unique dental condition, you may have several treatment options presented to you. You will be able to ask questions and make informed decisions.We have found this simple approach works very well to make sure everything is clear and understood before any treatment is performed.
